In order to obtain a certified birth or marriage/civil union certificate, you must provide the following information:
· Fill out the attached request form
· Provide copies of identification – a photo identification; two forms of identification showing current residence
*Married Female Applicants must provide a copy of their marriage certificate in addition to photo id and two forms showing current residents.
· Money Order (no personal checks) payable to Boonton Township in the amount of $10.00 (for each certificate requested)
· Return self-addressed stamped envelope
If you are applying for a certified copy of a death certificate, please note the following:
· Only a surviving spouse/partner, child, parent, funeral director or the attorney handling the estate can request a certified death certificate
· Copies of two forms of identification (one being a photo identification, one form showing current residence)
· If you are not the surviving spouse/partner, child, or parent, certified proof from the Court that you are the Executor/Executrix to the estate or have been appointed by the Surrogate’s Office to handle the estate is required
· In the event a funeral director is requesting the death certificate, please provide the following information
- Photo identification and form of identification proving that you are associated with the funeral home of record
· In the event an attorney is requesting the death certificate, the following is requested:
- Documentation from the Surrogate’s Office (current Surrogate’s Certificate) that they are handling the estate
- Two forms of personal identification of the attorney requesting the certificate
· Fill out the attached request form
· Money Order (no personal checks) payable to Boonton Township in the amount of $10.00 (for each certificate requested)
· Return self-addressed stamped envelope
